What to Expect (Job Responsibilities):
- Provide direct supervision and oversight for a multi-disciplinary team supporting members with mental health, substance use, and intellectual/developmental disabilities
- Ensure staff complete comprehensive assessments and care plans for members as care needs change
- Establish a team-based, person-centered approach to care coordination
- Monitor teams to ensure integration of care through a multi-disciplinary approach
- Familiarize staff with evidenced-based practices and translate that knowledge into effective workflows
What is Required (Qualifications):
- Bachelor’s degree or licensure as an RN, and five (5) years of experience in care management, case management, or care coordination for complex individuals with I/DD or TBI, or
- Master’s degree in a human services field or licensure as an RN, and three (3) years of experience in care management, case management, or care coordination
- Certification required if qualifying as a Registered Nurse
- Valid driver’s license
- Must reside within North Carolina and be able to travel within the catchment area as required

- A minimum of two (2) years of experience supervising a team
