What to Expect (Job Responsibilities):
- Conduct In-Home Health Evaluations, including medication and medical history reviews, and physical evaluations
- Engage with patients to build trusted relationships and enhance their health journey
- Provide diagnostic and preventive services as indicated, using an iPad and clinical workflow
- Travel locally and within licensed states as needed to perform evaluations
- Ensure a safe and comfortable environment for conducting evaluations in patients' homes
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Ability to cover primary locations, specifically Erie County/Buffalo, New York, and surrounding areas
- Experience in providing direct patient care in a healthcare setting
- Strong interpersonal and communication skills
How to Stand Out (Preferred Qualifications):
- Multi-state licenses
- Previous experience in in-home health evaluations or similar roles
- Familiarity with conducting virtual health visits
- Additional certifications in diagnostic and preventive services
- Commitment to continuous professional development and education
