      What to Expect (Job Responsibilities):
      - Conduct in-home health evaluations, including medication and medical history reviews, physical evaluations, and diagnostic tests as ordered
      - Build trusted relationships with patients to improve their health and connect them to appropriate care
      - Perform virtual visits as needed, ensuring accessibility for all patients
      - Travel locally and within licensed states to provide care in diverse home environments
      - Utilize an iPad and intuitive clinical workflow for efficient evaluations

      What is Required (Qualifications):
      - Active, unrestricted license(s) in coverage area(s)
      - Board certification as a Nurse Practitioner or Physician Assistant
      - Ability to cover primary locations, specifically Erie County/Buffalo, NY
      - Commitment to providing compassionate, patient-centered care
      - Flexibility to adapt to various home environments and conditions

      How to Stand Out (Preferred Qualifications):
      - Multi-state licenses preferred to enhance service coverage
      - Previous experience in home health evaluations or similar roles
      - Strong communication skills to effectively engage with patients
      - Ability to work independently and manage time efficiently
      - Familiarity with diagnostic and preventive services
