To help further the mission of Harbor, we are looking for an talented Pharmacist to join our Care Teams as we enhance our scope of services for the elderly in our communities.

      Role:

      Under the direction of the Harbor Health Elder Service Plan (ESP) Medical Director, the Pharmacist is responsible for enhancing the care of Elder Service Plan/PACE participants with multiple chronic co-morbid conditions requiring complex medication regimens. The Pharmacist is responsible for medication reconciliation during transitions of care including potential treatment optimization and coordination of care with designated pharmacies. The Pharmacist will work to increase medication adherence and reduce hospitalizations through recommendations for appropriate packaging and medication access and delivery options. Additionally, the Pharmacist will collaborate with the Pharmacy Benefit Manager (PBM) to ensure adherence to Medicare Part D regulations and cost-effective medication utilization. The Pharmacist will work closely with the clinical pharmacist as well as primary care providers to develop and implement strategies which continuously improve the quality of care at the best value for participants and the organization.

      Responsibilities:

      Medication Reconciliation and Quality Control

      • Performs medication reconciliation during transitions of care for new and established patients including documentation of all current medications, assessing adherence, determining appropriate medication packaging and delivery methods, identifying potential medication interactions, and verifying allergies to be entered in the Electronic Medical Record (EMR)
      • Participates in transitional planning for patients moving or disenrolling from PACE
      • Ensures creation, support, maintenance, and updates to patient-specific medication profiles including delivery and packaging information
      • Monitors medication storage in the PACE center and use of the automated dispensing machines.
      • Oversees medication incident reporting functions including follow-up of patient-specific incidents, preparation of reports for Quality Manager, and continuous quality improvement recommendations.

      Pharmacy Liaison

      • Serves as operational liaison and primary contact between IDT and pharmacies, including instances where a new pharmacy is engaged for specific patient needs
      • Works closely with IDT members to optimize medication safety, compliance and adherence and provides recommendations based on individual patient needs
      • Promotes internal and external customer satisfaction and service according to the PACE model of care
      • Ensures compliance to state and federal laws and applicable rules and regulations required for pharmacy practice.
      • Reports and works to rectify or prevent any medication incidents, HIPAA violations, Fraud, Waste and Abuse or other problems or violations.
      • Develops, writes and iterates on PACE pharmacy Standard Operating Procedures (SOPs) to ensure consistent workflow, particularly for nuanced or infrequent pharmacy processes.

      Managed Care

      • Serves as primary contact for the PBM, including troubleshooting claims issues, overriding/authorizing medications, and understanding processes
      • Reviews reports from PBM to confirm accuracy, educate providers to improvement opportunities, and respond to PBM clinical requests such as medical necessity, Medicare Part B or D determinations, and other reports as needed
      • Provides subject matter expertise to IDT about pharmacy claim processing issues to support issue resolution
      • Maintains an awareness of drug costs and works with finance on developing an accurate budget and Medicare Part D bid
      • Reviews PBM data to ensure accurate billing of prescription orders
      • Participates in Medicare Compliance committee and is actively engaged in fraud, waste, and abuse audits

      Secondary Duties

      Assists Clinical Pharmacist with the following as needed

      • Reviews patients’ medication profile and makes recommendations to enhance patient outcomes or safety as needed. Ensures cycle, delivery, and administration data is correct in the EHR.
      • Monitors prescribing practices across all PACE sites to ensure pharmacy and formulary management initiatives, and processes are uniformly implemented with appropriate education provided.
      • Supports center or community nursing by administering vaccines.

      Requirements:

      • PharmD or Bachelor’s degree with equivalent experience required
      • Current Registered Pharmacist License with the State of Massachusetts required
      • Current BLS (Basic Life Support for Healthcare Providers) Certification required
      • 1 year of experience working with a frail or elderly population preferred
      • Board Certification in Geriatrics preferred
      • Intermediate to Advanced skills in Microsoft Word, Excel and Outlook
      • Excellent verbal and written communication skills
      • Experience with NEXTGEN EMR preferred
      • Must be able to travel in a timely manner to all ESP and community sites.
