What to Expect (Job Responsibilities):
- Coordinate the scheduling and administration of Long Acting Injectables (LAIs) for patients
- Follow up with clients recently discharged from the hospital or Emergency Department for mental health concerns
- Perform telephone outreach to clients and service providers to facilitate care gap outreach
- Provide instruction, training, and health information based on clients' care plans
- Ensure compliance with state and federal privacy regulations, including HIPAA
What is Required (Qualifications):
- Successful completion of an accredited nursing program leading to an AD, BS, or AA degree
- Current RN license in the State of Oregon
- At least one (1) year of nursing experience in care management, telephonic client care, or population health management
- Must be a resident of Oregon and live in the state
How to Stand Out (Preferred Qualifications):
- Experience with telephonic patient care and population health management
- Familiarity with computerized client data information systems
- Experience in managing sensitive and confidential information
