      Job summary

      To be responsible for the accurate and timely processing of prescription requests for the entitled population, liaising effectively with all relevant personnel. Duties can include but are not limited to, the processing of repeat prescriptions in electronic and hard copy format, including online requests. In addition, the post-holder will be required to support the multidisciplinary team, ensuring the strategic objectives of the practice are met.

      Main duties of the job

      Process repeat prescription requests in readiness for GP signature within agreed timeframe.

      Reconcile medications to facilitate collection timetable.

      Liaise with patients and Clinician's regarding medication queries

      Ensure medication reviews are arranged by maintaining an accurate recall system

      Effectively liaise with external services i.e. district nurses to ensure medicaments are arranged for housebound patients

      Update patient records as appropriate

      Record and reported significant events accurately and appropriately, informing the Clinical Pharmacist immediately following internal reporting procedures

      Primary Responsibilities

      The following are the core responsibilities of the prescription clerk. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

      a. Process repeat prescription requests received by post, repeat prescription re-order form and the online patient access system, in accordance with practice protocol

      b. Produce prescriptions for the GP to sign in an acceptable time frame

      c. Ensure all prescription requests are processed and are ready for collection within a 48-hour time frame

      d. Generate requests for medication on behalf of the patient for non-repeat items, where this has been agreed by the prescribing clinician.

      e. Actively encourage patients to reconcile medications to facilitate monthly collections

      f. Discuss with patients their requirements, encouraging patients to order only what they require

      g. Act upon compliance issues, liaising with the appropriate clinician

      h. Ensure medication reviews are arranged by maintaining an accurate recall system

      i. Effectively liaise with external services i.e. district nurses to ensure medications are arranged for housebound patients

      j. Input data into the patients healthcare records as necessary

      k. Deal with all prescription related enquiries in a timely manner

      l. Record and reported significant events accurately and appropriately, informing the Management Team immediately

      m. Read code data on EMIS Health Care System

      n. Carry out system searches as requested

      o. Maintain a clean, tidy, effective working area at all times

      Secondary Responsibilities

      In addition to the primary responsibilities, the Prescription Clerk may be requested to:

      a. Support the practice Clinical Pharmacist as required

      b. Partake in audit as directed by the audit lead

      c. Support the Management Team in reviewing and updating the practice prescribing policy
