      What to Expect (Job Responsibilities):
      - Serve as a case manager and health coach, utilizing disease management and remote patient monitoring technologies
      - Collaborate with healthcare teams to assess and plan healthcare options for Veterans
      - Advocate for Veterans' healthcare needs and coordinate services to meet those needs
      - Monitor and evaluate the effectiveness of implemented healthcare plans
      - Engage in processes that ensure the delivery of quality care and support

      What is Required (Qualifications):
      - Must hold an active Registered Nurse (RN) license
      - Proven experience in case management and health coaching
      - Familiarity with disease management and healthcare informatics
      - Strong communication and interpersonal skills
      - Ability to work independently and collaboratively in a remote setting

      How to Stand Out (Preferred Qualifications):
      - Experience working with Veterans or within the Veterans Affairs healthcare system
      - Knowledge of remote patient monitoring technologies
      - Certification in case management or related field
      - Previous experience in telehealth or home healthcare settings
