Job summary
We are seeking enthusiastic and dedicated Occupational Therapist to join our dynamic and friendly inpatient rehabilitation team at Petersfield Community Hospital.
You will work alongside a dynamic multidisciplinary team to deliver high quality care and rehabilitation. You will use your skills and experience to provide individualised rehabilitation programmes to patients with a range of functional and rehabilitative needs, who are either recovering from an acute episode of care or step up from the community. Focus will be on innovative rehabilitation programmes that enable safe discharge with ongoing plans being met through our community integrated enablement services.
You will have a clear understanding of patients with long term conditions and should be able to demonstrate a high level of clinical assessment and reasoning, as well as being able to demonstrate experience of rehabilitation through group work and individualised programmes.
You will be a highly motivated individual with excellent communication skills. You will need to be confident and assertive with excellent personal skills and have the ability to demonstrate these in a pressurised environment.
You will be Educated to Degree or Diploma level and hold HCPC registration. The successful applicant will be proficient using IT systems.
We will consider staff looking for part time or job share opportunities. Access to a vehicle and possession of a full driving license is essential.
Main duties of the job
To provide advanced clinical and professional Occupational Therapy to the Community Therapy Team/Community Hospital Team, and to provide support to the Integrated Community Care team leaders in ensuring that the team contributes to the implementation of Southern Health Clinical Governance Strategies.
Undertake Occupational Therapy assessments of patients with diverse and complex conditions, providing a clinical diagnosis (where appropriate), development, and delivery of individualised treatment programmes.
To support the case management process by providing expert clinical advice and therapy to patients with a variety of complex conditions living in the community who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission to secondary care
To provide clinical Occupational Therapy skills within the Community Therapy Team/ Community Hospital Team(s), and all other provider organisations.
To be skilled in assessing and treating a range of complex factors relating to the patient's condition, diagnosis and prognosis
