      Are you interested in social services work that is dedicated to improving the lives of seniors? As the Social Services Designee at The Laurels of Gahanna you will provide medically-related social services that help guests to attain and/or maintain their physical, mental and psychosocial well-being. The Social Services Designee is responsible for assisting and coordinating with the facility's Social Service Department to streamline the process for discharge planning.

      Responsibilities:

      • Meeting the resident/guest on or shortly after the admission and following them until they discharge to the community.
      • Attend the 72 hour care plan meeting.
      • Ordering equipment for the community setting.
      • Arranging home health nursing for after discharge.
      • Coordinating the medications for community use.
      • Scheduling IV home services if necessary.
      • Communicate with external case managers, as needed.
      • Coordinate the entire discharge, with IDT involvement.
      • Attending care conferences for sub-acute residents/guests.
      • Develop an individualized discharge care plan for the resident/guest discharge in collaboration with the facility, the family, and the practitioner, assuring the resident/guest receives the care that will meet their needs in the community.

      Education and/or Experience:

      • High school diploma/GED
