- Attend the 72 hour care plan meeting.
- Ordering equipment for the community setting.
- Arranging home health nursing for after discharge.
- Coordinating the medications for community use.
- Scheduling IV home services if necessary.
- Communicate with external case managers, as needed.
- Coordinate the entire discharge, with IDT involvement.
- Attending care conferences for sub-acute residents/guests.
- Develop an individualized discharge care plan for the resident/guest discharge in collaboration with the facility, the family, and the practitioner, assuring the resident/guest receives the care that will meet their needs in the community.
- High school diploma/GED

As the Social Services Designee at The Laurels of Gahanna you will provide medically-related social services that help guests to attain and/or maintain their physical, mental and psychosocial well-being. The Social Services Designee is responsible for assisting and coordinating with the facility's Social Service Department to streamline the process for discharge planning.
