  • Job Description

      Req#: REF2896T

      Company Description

      • Start Date: we have a New Employee Orientation starting soon
      • Salary: $60-70k
      • Remote: this is a hybrid position. You will be required to be in the facility frequently, therefore you will need to live within the Chicagoland area.
      • This is a “Road Warrior” position. Meaning, you will be interfacing daily with our regional referral/lead sources. This is an external business getting position. You will be in the hospitals (and other locations/centers) every day to secure new admissions to build census.

Community Care Center is a large, inner-city skilled nursing facility that provides care across multiple levels—from geriatric long-term care to a robust behavioral health program. Our patients range from their mid-40s to 80+, many living with physical, cognitive, or behavioral health conditions.

      • Our staff and patients are known for their warmth—you’ll be greeted with smiles and “hellos” as you walk through the halls.
      • We’re easy to reach, located near W. 43rd & State Street, just off 90/94 and a few blocks from the train.

      Job Description

About the position: Our Marketing Manager is intertwined with admissions at the facility level by performing all out-bound lead generating activities.

You will be responsible for daily Medicare/Managed Care census-building activities of one of our skilled nursing facilities in Chicago (in the Bronzeville Park neighborhood). Depending on your success, you may also (in the future) pick up responsibilities for other centers in the Chicago market. Some of these responsibilities will include:

      • Create & engage in out-bound business getting events/activities for 1-3 SNF & ICF locations
      • Manage relevant CRM metrics to ensure measurable results are attained
      • Successfully build the Medicare census, while maintaining our custodial care levels
      • This position will not have any direct reports, but will be intertwined with administration, central admissions & our regional marketing team for locations throughout Chicagoland.
      • This is a “Road Warrior” position; meaning, you will be out interfacing daily with our referral sources. (One moment this may include visiting with patients at the bed-side and the next you could be hosting events for a group of discharge planners.)
      • This is an external business getting position, not administrative. You will be in the hospitals (and other sources) every day.

      Qualifications

      What we need from you:

      • Marketing experience within the skilled nursing facility (SNF or ICF) industry. (You will be expected to rapidly impact our census.)
      • Strong relationships with existing referral sources you can immediately capitalize on/bring with you.
      • Knowledge of various custodial payor sources, including Managed Medicaid/Medicare, and of the overall admissions process
      • Previous experience with Point Click Care is helpful
      • Associates or Bachelor’s Degree in a relevant healthcare field is ideal so that you can identify appropriate admissions (example: LPN, RN, LSW, RT ...)

      Additional Information

Please include a cover letter outlining prior metrics and accomplishments.

Note: This job description is not intended to be an exhaustive list of all duties, responsibilities or qualifications associated with the job.
