What to Expect (Job Responsibilities):
- Educate members on managing their health, including topics such as nutrition, safety, and mindfulness
- Assist members in engaging with healthcare providers and care management teams regarding their health decisions
- Document all member interactions accurately in the appropriate records
- Demonstrate empathy and understanding of the challenges faced by members and their caregivers
- Follow program workflows and provide follow-up support to members regarding their personal action plans
What is Required (Qualifications):
- At least 2 years of experience in the Health and Wellness field
- Experience in care management, healthcare, social work, or similar backgrounds
- Associate's Degree required; Bachelor's preferred
- Health Coach Certification
- Ability to document and track member interactions effectively
How to Stand Out (Preferred Qualifications):
- Bilingual proficiency
- Experience in Community Health or Public Health
- Knowledge of Managed Care and Medicare systems
