      What to Expect (Job Responsibilities):
      - Perform outreach to members via phone to assist with care plan next steps and resources
      - Serve as the primary contact for members and providers, addressing inquiries and concerns
      - Support member onboarding by sending welcome letters and educational materials
      - Document and maintain non-clinical member records in compliance with state regulations
      - Connect members to community resources to ensure high-quality care and support

      What is Required (Qualifications):
      - High School diploma or GED
      - 1-2 years of related experience in a healthcare or administrative role
      - Ability to explain care plan procedures and protocols
      - Strong communication skills for member and provider interactions
      - Knowledge of local benefits and resources related to healthcare services

      How to Stand Out (Preferred Qualifications):
      - Experience in care management or customer service within the healthcare sector
      - Familiarity with Social Determinants of Health (SDOH) and community resource navigation
      - Proficiency in documenting and maintaining healthcare records
      - Strong problem-solving skills to address member inquiries effectively
