- Acting as the primary point of contact for patients, families, and healthcare professionals regarding discharge and care transitions.
- Assessing patient needs and developing tailored discharge plans that align with clinical recommendations and community resources.
- Liaising with internal teams and external agencies to facilitate timely referrals and continuity of care.
- Monitoring progress, addressing barriers to discharge, and escalating issues where necessary to maintain patient flow.
- Supporting quality improvement initiatives and compliance with hospital policies and national standards.

- Use OPTICA to monitor patient status, track discharge readiness, and update care plans in real time.
- Maintain and update the Wardboard daily to ensure accurate visibility of patient progress and discharge priorities for the multidisciplinary team.
- Act as the key liaison between patients, families, clinical teams, and external agencies to facilitate safe and timely discharge.
- Identify and address barriers to discharge, escalating issues where necessary to maintain patient flow and meet hospital targets.
- Arrange referrals and community support services, ensuring continuity of care post-discharge.

- GCSE grade C or above (or equivalent) in English and Maths
- IT Qualification
- Ability to demonstrate a working knowledge of current NHS guidelines/targets/initiative relating to patient flow
- Ability to demonstrate an interest and enthusiasm in the Case Manager role
- Ability to work flexibly to meet the needs of the service
- Excellent communication skills and the ability to interact appropriately with patients, relatives and multi -disciplinary team.
