      What to Expect (Job Responsibilities):
      - Conduct psychosocial assessments and re-assessments to develop and update individualized service plans
      - Provide supportive counseling and intensive life skills training to individuals and client groups
      - Serve as an advocate to identify and assess entitlement resources and social services
      - Maintain accurate client case records and submit required program forms and documentation
      - Communicate effectively with internal and external programs and community resources

      What is Required (Qualifications):
      - Bachelor’s degree in Social Work, Psychology, or related human services field from an accredited institution
      - One year of experience in a related field preferred
      - Knowledge of financial entitlement programs and available social services
      - Must obtain Criminal History Clearance, Child Abuse Clearance, and FBI Fingerprint Clearances
      - Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program

      - Demonstrated ability to obtain vital information during interviews for proper recommendations
      - Ability to safely operate a variety of agency vehicles
      - Experience assisting clients with physical limitations
      - Skills in inspecting and recognizing violations pertaining to program regulations
