      Coastal Healthcare is looking for a Community Health Nurse to assist in providing direct patient care and treatment in our primary care clinics, VA, or in the School-based Health Centers (SBHC). The person in this position promotes health through education, care coordination and health maintenance. This is a very important role for us and we're looking for an experienced and passionate healthcare professional to join our team.



      $40.19 - $54.02 per hour*

      Community Health Nurses will follow the six standards of practice:

      • Assessment – Collect comprehensive data related to the health of the population served. Document assessment data in terms that are understandable to all involved in the care process.
      • Diagnosis and Priorities – Analysis of the assessment data to derive a nursing diagnosis and priorities. Document diagnoses or concerns in a manner that facilitates the population’s involvement in developing a plan of care.
      • Outcome identification – Based on the diagnosis the CHN will identify outcomes that are culturally relevant, health literate, based in evidence and attainable through local resources and policies. Document expected outcomes and measurable objectives.
      • Planning – Develop a plan that is client-centered and reflects best practices by identifying strategies, action plans and alternatives to meeting expected outcomes. Document the plan using health literate and culturally competent language that can be understood by care team and population being served.
      • Implementation – Oversee the implementation of the plan through treatment, case management, referrals to community partners, collaboration with community resources, health promotion and education. Document activities and status of care coordination.
      • Evaluation – Apply on-going assessment data to revise plans, interventions and activities as appropriate for ultimate health outcomes for the population. Document changes to enhance effectiveness of interventions.

      For Primary Care and School-based Health Center CHN2 will perform additional job responsibilities:

      • Apply theories and principles of primary care and adolescent health nursing practices. This includes services in the clinics and schools.
      • Consistently set appropriate priorities in providing quality patient care that is organized and time efficient. Utilize staff feedback in patient care. Maintain and ensure confidentiality of patient’s conditions, records, and other data on a consistent basis.
      • Perform direct care activities by administering medications, observing, and documenting patient responses and/or side effects. Administer interventions according to established standing orders and protocols.
      • Provide an environment that demonstrates caring, compassion, and respect for all patients and families. Create and maintain a high-performance clinic environment characterized by positive role model, leadership, and a strong team orientation.
      • Communicate, as necessary, with other clinic and departmental personnel to coordinate efforts, resolve operational problems, and provide high quality, comprehensive patient care.
      • Meet training requirements and practices according to HRSA, HHS and OHA guidelines.
      • Participate in on-going quality improvement and reports from data systems to guide and improve practice.
      • Establish professional rapport with clients, community, and health care partners to promote health.

      Other duties as assigned

      • Attend and participates in team meetings, trainings, and activities to further the health of the community.

      Minimum Education Level

      Associate degree in Nursing from an accredited school of nursing.


      Minimum Experience Level*

      Three years of nursing experience.

      *Practicums in Nursing programs and lived experience may substitute for years of experience.


      Driver’s License

      Oregon Driver's License required within 30 days of hire


      Other Licenses

      Must possess a valid license to practice in the State of Oregon as a Registered Professional Nurse as defined by statutes or administrative rules, which include but are not limited to ORS 678.010 to 678.360 at the time of appointment. The employee is required to maintain current and active licensures and certifications, as applicable, and to provide the appropriate verifications at the time of renewals.


      Desirable Qualifications: Bachelor of Science degree in Nursing (BSN) from an accredited school of nursing with one year of nursing experience, preferably in a primary care position.


      Knowledge of:

      • The principles, practices, methods, and procedures of nursing as applied to community and public health.
      • The uses and effects of medications, immunizations.
      • Issues of alcohol and drug abuse.
      • The strength-based model of behavior change.
      • Problem-solving techniques and negotiation skills in accordance with motivational interviewing and behavior change theory.
      • Utilization of community resources.
      • Medical record keeping practices.
      • Functions and resources of private and public agencies.
