      Job summary

      The Department of Healthcare for Older People provides a comprehensive service for older patients both in hospital and in community settings and leads on the care of older people on four sites: Birmingham Heartlands Hospital, Good Hope Hospital, Queen Elizabeth Hospital Birmingham and Solihull Hospital. We are committed and passionate about older patients living with frailty.

      The speciality wishes to recruit additional Consultant Geriatricians at all four hospitals: these are a combination of new and replacement posts. We welcome and can accommodate any subspecialty interest, though we particularly wish to recruit consultants with an interest in peri-operative geriatrics (Orthogeriatrics and surgical liaison), movement disorders and acute geriatrics. Further description of the specific services and areas for development are detailed in the Job Description.

      Main duties of the job

      CLINICAL CARE

      Care of inpatients

      Administrative duties related to patient care

      Contribution to undergraduate, post-graduate and continuing medical education activity, including AHPs as part of the multidisciplinary team.

      Regular participation in departmental and directorate meetings.

      Consultants are encouraged and supported with research projects.

      Provision of cover for Consultant colleagues periods of leave in accordance with arrangements agreed within Trust policy.

      To help the department and Trust in emergencies.

      Appropriate evening and weekend on-call commitments for acute medicine and specialty.

      Job responsibilities

      Queen Elizabeth Hospital Birmingham (QEHB)

      Services unique to QEHB site include:

      *Further development of the peri-operative geriatrics service aiding specialist input into emergency general surgical and silver major trauma patients. An additional role will be establishing a geriatrician in the newly formed multi-disciplinary Extended Post-operative Care Units and expansion of the peri-operative service to tertiary surgical specialities such as cardiothoracics and neurosurgery.

      *Expansion of the complex regional Parkinson's disease clinic with links to the neurosciences and neurosurgical departments for interventions such as Deep Brain Stimulation.

      *Increasing development of academic geriatric medicine, linked to the Institute of Inflammation and Ageing at the University of Birmingham. Regular NIHR academic clinical fellows and lecturers, with an increasing portfolio of investigator led studies, focussing on biological mechanisms of ageing, multi-morbidity and sarcopenia.

      *Support to inpatients at Birmingham Community Healthcare NHS Foundation Trust and Norman Power Centre providing specialist geriatric expertise to the city's population. Opportunities at West Heath and Mosely Hall Hospital include stroke and trauma rehabilitation whilst the focus at Norman Power Unit is to support the best route out of hospital for patients who require a more appropriate care setting whilst awaiting the next stage in their care.

      *Unique opportunity to be involved in the expansion of OPAL+, a collaborative partnership between the OPAL team at QEHB and the West Midlands Ambulance Service. The service launched in March 2020 and since then crews have used video technology to avoid unnecessary transfers to hospitals. OPAL+ has close links to the Early Intervention and Community Palliative Care Teams and is looking forward to joint working with the Community Mental Health Team in the near future.

      Birmingham Heartlands Hospital (BHH)

      Services unique to BHH site include:

      *The opportunity to join a well-established team of front door/interface geriatricians to improve the patient's journey in hospital. The post holder will have a chance to take part in service development to establish an acute admissions assessment unit for frail older adults as well as take part in our OPAL service (older people's assessment and liaison service), an MDT team that provides CGA in ED and the medical assessment unit.

      *The opportunity to develop a community service with inreach to Intermediate Care centres and developing a rapid access GP clinic to provide early review of community based patients and avoid hospital admissions where possible.

      *Provision of a liaison Orthogeriatrics service -- there is a centralised fracture neck of femur service for all patients across the trust based at the BHH site, giving an excellent opportunity to develop orthogeriatric services at the site. MDT falls clinics and osteoporosis clinics also form part of orthogeriatric services and there is opportunity to develop tilt table testing and syncope clinics if the candidate has the expertise.

      Good Hope Hospital

      Services unique to Good Hope Hospital include:-

      *Opportunity to further develop interface/acute geriatric medicine services at the site which have recently undergone a significant expansion. The OPAL multidisciplinary model is now firmly embedded at the Good Hope site, with excellent links now built between OPAL and community teams within Birmingham and South Staffordshire. Developments currently planned include establishing a virtual link between the OPAL team and local intermediate care centres, developing the use of technology to support patients within these centres, and building links with GPs to provide enhanced support to frail patients in the community.

      *Community wards based on the Good Hope site provide an excellent opportunity for consultants wishing to develop links between acute and community wards, and build further relationships with community partners and drive forward the progress made already with early supported discharge pathways.

      *Opportunity to lead Parkinson's disease service, developing and expanding the service available to patients with Parkinson's disease on this site
