What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations
- Utilize an iPad and intuitive clinical workflow for assessments, ensuring high-quality care delivery
- Provide diagnostic and preventive services as indicated, such as spirometry and bone density screenings
- Engage with patients to answer health-related questions and provide education on health management
- Travel locally within licensed states to conduct evaluations and virtual visits as necessary
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Ability to cover primary locations, including the Bronx, New York, and surrounding areas
- Flexibility to travel locally and conduct virtual visits as needed
- Experience in conducting health evaluations or similar clinical assessments

- Previous experience in in-home health evaluations or community health settings
- Strong interpersonal skills to build rapport with patients
