What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication reviews and physical assessments
- Build trusted relationships with patients to improve their health outcomes
- Utilize technology, including an iPad, to facilitate clinical workflows during evaluations
- Provide diagnostic and preventive services as indicated
- Travel locally within licensed states as needed to complete evaluations
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification is required
- Ability to cover primary locations, specifically Suffolk County, New York
- Flexibility to travel locally and within licensed states
- Prior experience in providing in-home healthcare evaluations is essential
How to Stand Out (Preferred Qualifications):
- Multi-state licenses are preferred
- Experience in conducting health evaluations in diverse environments
- Familiarity with diagnostic and preventive services
- Strong interpersonal skills for patient engagement
- Previous work in a healthcare setting focused on patient-centered care
