JOB OVERVIEW
Always Best Care Senior Services is a well-established provider in the community for over 10 years, now expanding our services to include Medicare-certified home health care. We are seeking an experienced Licensed Vocational Nurse (LVN) to join our growing home health clinical team.
This position will begin as Per Diem, with the opportunity to quickly transition to Full-Time as patient census grows.
PAY RANGE
$40 – $45 per hour (Per Diem)
RESPONSIBILITIES
• Provide skilled nursing visits in patients’ homes per RN-established plan of care
• Perform patient assessments, home safety evaluations, and environmental checks
• Administer treatments using appropriate aseptic techniques
• Educate patients and families on self-care, disease management, and safety
• Document visits accurately per Medicare and agency standards
• Communicate changes to supervising RN
• Participate in case conferences and interdisciplinary collaboration
QUALIFICATIONS
• Graduate of an approved LVN program
• Active LVN license in California (good standing)
• Minimum of 1 year home health experience required
• CPR certification
• Reliable transportation
PREFERRED EXPERIENCE
• Medicare-certified home health experience
• WellSky EMR experience or familiarity
