What to Expect (Job Responsibilities):
- Develop, assess, and coordinate holistic care management activities for long-term care members
- Assist in creating personalized service care plans for members and educate them and their families on available services and benefits
- Coordinate between members, families, and care providers to ensure accessibility of services
- Monitor care plans and member status, providing recommendations based on identified needs
- Collect and maintain member information to ensure compliance with relevant regulations
What is Required (Qualifications):
- Bachelor's degree and 1 year of related experience, or equivalent experience
- For Iowa Plan Only: Bachelor's degree and 1+ years of experience with the populations served, or RN with 6+ years of relevant experience
- Strong knowledge of long-term care services and care management
- Excellent communication skills for effective interaction with members and healthcare providers
- Ability to document and maintain accurate member information

- Experience working with long-term care populations
- Familiarity with state and federal healthcare regulations
- Skills in providing education and support to members and their families
- Previous experience in care coordination or management
- Knowledge of community resources available for long-term care members
