What to Expect (Job Responsibilities):
- Conduct in-home assessments and outpatient clinical work on health plan members
- Obtain comprehensive patient histories, perform physical exams, and conduct medication reviews
- Facilitate end-of-life care discussions and laboratory specimen collection in the home setting
- Identify diagnoses for active medical management and chronic disease management
- Communicate assessment findings to primary care providers to address potential gaps in care
What is Required (Qualifications):
- Active California state Nurse Practitioner or Physician Assistant license
- Master's degree from an accredited Nurse Practitioner or Physician Assistant program
- Strong communication skills, both verbal and written
- Effective problem-solving, organizational, and time management skills
- Ability to work in a fast-paced environment
How to Stand Out (Preferred Qualifications):
- One (1) year of prior clinical or home care experience
- Previous experience with electronic medical records (EMR)
- Experience in caring for older adult (geriatric) patients
- Active RN License and Furnishing number for Nurse Practitioners
- Valid BLS certification and NPI Number
