  Moorings Park is looking for a Home Health Physical Therapist Assistant. The licensed Physical Therapist Assistant provides physical therapy services to clients under the supervision of the Physical Therapist. He/she carries out the treatment plan delineated by the PT, assists the PT in resident treatment and carries out departmental duties as assigned. The P.T.A. shall not evaluate residents nor can he/she initiate any changes in a resident's program unless specifically directed to do so by the PT. P.T.A.'s may perform screenings at the discretion of the supervising PT.

  • Provides Physical Therapy Assistant provides patient services as assigned and is directly supervised by the Physical Therapist.
  • Assist the PT in the provision of direct patient treatment which includes:
    1. Treat with active and passive exercises, muscle re-education, gait and functional training.
    2. Administer therapy modalities as prescribed by physician. Monitor plan for effectiveness.
    3. Arrange for more continued Physical Therapy services, when indicated, with a Physical Therapy out-patient facility.
    4. Observe and report patient's progress or lack of progress to the physician.
    5. Communicate with interdisciplinary team regularly to coordinate care.
    6. Educate patient and/or family regarding therapy program and safety.
    7. Fit, evaluate and adjust adaptive devices.
    8. Prepare patient/family for discharge.
    9. Document, accurately and timely, the patient's treatment, care, evaluation, and progress and communications with other staff members.
    10. Participates in in-services, the QAPI program and patient care conferences.
    11. Attends in-services and assumes responsibility for continuing education and maintaining current knowledge.
  • Works with confidential data disclosure of which would be contrary to the best interests of the facility and professional ethics. Actively promotes resident confidentiality and rights.
  
  
  Job Requirements:
  
  • Excellent oral and written communication skills in the English language required
  • During emergencies or natural disasters may be required to be on duty for the duration of the disaster.
  • Graduate of an accredited school for Physical Therapist Assistant.
  • Current registration with the American Physical Therapy Association; Meet CEU and HIV licensure requirements.
  • CPR certification optional.
