- Assessing and implementing social work services in outpatient setting
- Assist department team with difficult and complex social situations and provides social work interventions as appropriate
- Provide education and contact information to patients and families regarding needed community resources
- Provide psychosocial assessments and interventions to improve or maintain the social, emotional, and physical health status of the patient
- Completes comprehensive psychosocial assessments including functional status, cognitive ability, support systems, available resources, and barriers to care
- Ensures documentation of assessment, plan, and interventions are included in the medical record
- Maintains professional accountability by maintaining patient privacy, confidentiality, and health through advocacy and adherence to ethical, legal, accreditation, certification, and regulatory standards or guidelines
- Education: Graduate of a school of social work that is approved by the Council on Social Work Education with a Master’s Degree (MSW), required
- Experience: Minimum of two years professional social work experience in a healthcare, hospital, or community health setting providing clinical and counseling services, required. Experience with patients with chronic conditions is a plus.
- Licensure/Certification:
- LCSW or LCSW-A is required. Hours and supervision towards licensure may not be available for this role.
- Additional Skills (required): Ability to successfully complete generic and department specific skills validation and competency testing; knowledge of chronic disease issues; knowledge of community resources; knowledge of department processes and needs. Demonstrate above average ability in the following areas: managing a complex and varied work load, understanding physical and psychosocial assessment parameters. Strong written and verbal skills; strong problem solving, decision making, organizational and critical thinking skills; and ability to network and access appropriate resources.

What We OfferSchedule: Monday - Thursday 7:30 AM - 4:00 PM; Part-time with full time benefit eligibility
Location: Heart and Vascular Institute at Presbyterian Medical Center; Presbyterian is our 661 bed level 2 trauma center centrally located just outside of uptown Charlotte
This role is ideal for a social worker experienced in working in a healthcare setting looking to work in an outpatient environment with a collaborative team.
