      What to Expect (Job Responsibilities):
      - Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations
      - Engage with patients to answer health-related questions and provide education on health management
      - Utilize technology, such as an iPad, to facilitate clinical workflows during evaluations
      - Perform diagnostic and preventive services as indicated, such as spirometry and bone density screenings
      - Travel locally within licensed states and conduct virtual visits as needed

      What is Required (Qualifications):
      - Active, unrestricted license(s) in coverage area(s)
      - Board certification in the relevant field
      - Ability to cover primary locations, including Bridgeport and surrounding areas
      - Must be flexible to travel locally and adapt to various home environments
      - Experience in providing patient-centered care in diverse settings

      How to Stand Out (Preferred Qualifications):
      - Multi-state licenses are preferred
      - Previous experience as a clinician in home health evaluations or similar settings
      - Familiarity with using technology for clinical assessments
      - Strong interpersonal skills and the ability to build trust with patients
      - Commitment to improving health outcomes in communities
