What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews
- Engage with patients to build trust and provide personalized health assessments
- Utilize technology for evaluations and follow clinical workflows as required
- Provide diagnostic and preventive services as indicated
- Adapt to various home environments to deliver care effectively
What is Required (Qualifications):
- Active, unrestricted license(s) in the relevant coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Ability to cover primary locations, including Cambridge and surrounding areas
- Willingness to travel locally and within licensed states as needed
- Flexibility to conduct virtual visits when necessary
How to Stand Out (Preferred Qualifications):
- Multi-state licenses are preferred
- Experience in providing in-home health evaluations or similar services
