What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews and physical assessments
- Build trusted relationships with patients to enhance their health outcomes
- Provide Diagnostic & Preventive Services as indicated, such as spirometry and bone density screenings
- Flexibly travel locally and within licensed states to conduct evaluations
- Perform virtual visits when necessary
What is Required (Qualifications):
- Active, unrestricted nursing or physician assistant license in the coverage area
- Board certification in the relevant field
- Ability to cover primary locations, including Concord and surrounding areas
- Experience in conducting health evaluations or similar clinical responsibilities
- Must be able to perform duties in diverse home environments

- Multi-state licenses
- Experience with in-home patient care or community health initiatives
- Strong communication and interpersonal skills for engaging with patients
- Willingness to adapt to varied home conditions and patient needs
- Familiarity with diagnostic and preventive health services
