What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews, physical assessments, and diagnostic tests as ordered
- Build trusted relationships with patients to enhance their health and bridge gaps in care
- Utilize technology, including an iPad and clinical workflow tools, to facilitate evaluations
- Travel locally and within licensed states as required for patient visits
- Provide virtual consultations when necessary to meet patient needs
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Willingness and ability to cover primary locations, specifically Minneapolis and surrounding areas
- Flexibility to travel locally and within licensed states
- Minimum of 29.5 hours available weekly for part-time work
