What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews
- Build trusted relationships with patients to enhance their health outcomes
- Perform physical evaluations and additional diagnostic tests as ordered
- Travel locally and within licensed states as necessary for patient visits
- Utilize technology, including an iPad, to streamline clinical workflows
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Ability to cover primary locations, including Queens and surrounding areas
- Flexibility to conduct virtual visits as needed
- Commitment to providing compassionate care in diverse home environments
How to Stand Out (Preferred Qualifications):
- Multi-state licenses to enhance coverage capabilities
- Experience in providing in-home health evaluations or similar services
- Ability to engage effectively with patients from diverse backgrounds
- Strong communication skills to educate patients on health matters
