What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medical history reviews and physical evaluations
- Engage with patients to answer health questions and provide personalized health assessments
- Utilize technology, such as an iPad, to facilitate clinical workflow during evaluations
- Travel locally and within licensed states as needed for patient visits
- Perform virtual visits as required
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification is required
- Ability to cover primary locations, including Rochester and surrounding areas
- Experience in providing in-home care or similar health evaluations
- Flexibility to adapt to diverse home environments
How to Stand Out (Preferred Qualifications):
- Multi-state licenses are preferred
- Previous experience as a Nurse Practitioner or Physician Assistant in a home health setting
- Strong interpersonal skills with the ability to build rapport with patients
- Familiarity with diagnostic and preventive services offerings
- Commitment to improving health outcomes in community settings
