What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews, and physical evaluations
- Build trusted relationships with patients to improve their health outcomes
- Utilize digital tools such as an iPad to streamline clinical workflows during evaluations
- Perform additional diagnostic tests as ordered by health plans, such as spirometry and diabetic eye exams
- Provide services as indicated, including preventive care and diagnostic services
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Ability to cover primary locations, specifically Springfield, MA
- Flexibility to travel locally and within licensed states as needed
- No prior administrative responsibilities required, allowing focus on patient care
How to Stand Out (Preferred Qualifications):
- Multi-state licenses are preferred
- Experience in in-home health assessments is beneficial
- Familiarity with digital clinical workflows and telehealth practices
- Strong interpersonal skills to engage with diverse patient populations
- Ability to adapt to various home environments and conditions
