      What to Expect (Job Responsibilities):
      - Initiate outreach to patients and caregivers to explain the benefits of home-based care services
      - Educate patients using bilingual English-Korean fluency, answering questions and collecting consent
      - Build rapport and guide decision-making using motivational interviewing techniques
      - Manage referrals end-to-end, ensuring timely outreach and follow-up with patients
      - Maintain accurate EMR documentation of outreach, patient concerns, and enrollment status

      What is Required (Qualifications):
      - Bilingual in English and Korean (reading, writing, speaking) with strong communication skills
      - 1-3 years of experience in healthcare outreach, patient education, or other patient-facing roles
      - Familiarity with EMR systems, preferably Athena, and motivational interviewing techniques
      - High school diploma or equivalent; an associate or bachelor's degree in healthcare, communications, or social work is preferred
      - Mission-driven mindset to help vulnerable or underserved populations access quality care

      How to Stand Out (Preferred Qualifications):
      - Experience in home health, hospice, managed care, or chronic condition programs
      - Skills in translating medical information into patient-friendly language
      - Comfort in handling objections and navigating sensitive conversations with confidence and compassion
