      How you'll make an impact in this role

      • Chronic Disease Management: Provide chronic disease management support for value-based care contracts across Ascension markets, including patient education and care planning.
      • Patient Assessment: Conduct comprehensive telephonic assessments to identify medical, psychosocial, behavioral, and environmental needs affecting patient health outcomes.
      • Interdisciplinary Collaboration: Collaborate with an interdisciplinary care team (physicians, nurses, social workers, behavioral health providers, and community organizations) to coordinate patient-centered care.
      • Referral Management: Manage registry-based referrals generated through automated weekly reporting.
      • Training and Structure: Participate in an initial in-person training program, followed by a fully remote work structure, with reimbursed travel and related training costs.

      What minimum qualifications you'll need

      Licensure / Certification / Registration:

      • Registered Nurse obtained prior to hire date or job transfer date required. Licensure required relevant to state in which work is performed.
      • Case Manager credentialed from the American Case Management Association (ACMA) preferred.
      • BLS Provider preferred. American Heart Association or American Red Cross accepted.
      • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Licensure required relevant to state in which work is performed

      Education:

      • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

      What additional requirements you'll need

      • Valid RN Licensure: Requires valid licensure (RN). Additional state licensure as needed if the license is not compact.
      • 3–5 years of experience in care management, case management, social services, community health, or clinical practice.
      • Proficiency in electronic health records and care management software. Technologically proficient with the ability to effectively operate in a fully remote environment using virtual communication and cloud-based tools.
      • Certification in Case Management (CCM, ACM, or similar), and experience within managed care, population health, or value-based care environments, preferred
      • Familiarity with Medicare/Medicaid policies and utilization management principles.
