What to Expect (Job Responsibilities):
- Assist with coordinating resources for patients within the Kidney and Liver Transplant Social Work team
- Facilitate patient placements in institutional settings and arrange necessary referrals
- Provide clerical assistance, including faxing, copying, and coordinating paperwork for patient discharge
- Maintain communication with the healthcare team regarding patient status and needs
- Assist patients and families with emergency resources, housing, and financial applications
What is Required (Qualifications):
- Bachelor of Social Work or a Bachelor's Degree in the behavioral sciences AND two years of experience as a Social Work Assistant I or equivalent
- OR an Associate of Arts Degree in the behavioral sciences AND four years of work experience as a Social Work Assistant I or equivalent
- Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration

- Previous experience as a Social Work Assistant in a medical setting
- Strong multi-tasking and organizational skills
- Ability to work effectively with diverse teams and individuals
