What to Expect (Job Responsibilities):
- Coordinate with the clinical team for OASIS review and plan of care development
- Educate staff on OASIS documentation in compliance with CMS guidelines
- Perform case conferences to evaluate appropriateness of services and develop care plans
- Conduct quarterly record reviews and communicate findings to the Executive Director
- Assist in the planning and implementation of in-service and continuing education programs
What is Required (Qualifications):
- Registered nurse with current licensure to practice professional nursing in California
- OASIS certification is required prior to starting the position
- Minimum of two years of experience conducting quality OASIS reviews in a home health setting
- Graduate of a National League for Nursing accredited school of nursing; BSN preferred
- Demonstrated experience in star measures and value-based purchasing
How to Stand Out (Preferred Qualifications):
- Experience with Home Care Home Base (HCHB)
- Excellent observation, verbal, and written communication skills
