      What to Expect (Job Responsibilities):
      - Conduct in-home health evaluations, including medication and medical history reviews and physical assessments
      - Engage with patients to build trusted relationships and address their unique health needs
      - Utilize technology, including an iPad, to streamline the evaluation process
      - Provide diagnostic and preventive services as indicated, such as spirometry and bone density screenings
      - Travel locally within licensed states as required to meet patient needs

      What is Required (Qualifications):
      - Active, unrestricted license(s) in coverage area(s)
      - Board certification as a Nurse Practitioner or Physician Assistant
      - Ability to cover primary locations, including Boston and surrounding areas
      - Flexibility to travel locally and conduct virtual visits as needed
      - Strong commitment to improving health outcomes for patients

      How to Stand Out (Preferred Qualifications):
      - Multi-state licenses
      - Experience in providing home health evaluations or similar services
      - Familiarity with technology and clinical workflows for health assessments
      - Ability to engage with diverse populations in various home environments
      - Prior experience in a patient-centered care model
