What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews, and physical assessments
- Build trusted relationships with patients to enhance their health and connect them to necessary care
- Perform virtual visits as needed and travel locally within licensed areas
- Utilize technology such as an iPad for evaluations and follow prescribed clinical workflows
- Provide diagnostic and preventive services as indicated, including spirometry and bone density screenings
What is Required (Qualifications):
- Active, unrestricted license(s) in relevant coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Ability to cover primary locations including Boston and surrounding areas
- Flexibility to travel locally and within licensed states as required
- Commitment to providing compassionate and personalized care
How to Stand Out (Preferred Qualifications):
- Multi-state licenses preferred for broader coverage
- Previous experience in in-home health evaluations or similar settings
- Strong interpersonal skills to effectively engage with patients in diverse environments
- Familiarity with diagnostic and preventive service protocols
