What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations
- Engage with patients to build trusted relationships and address health questions
- Provide Diagnostic & Preventive Services as indicated, such as spirometry and bone density screening
- Utilize an iPad and intuitive clinical workflow for each evaluation
- Collaborate with a national network of clinicians dedicated to improving health outcomes
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Ability to cover specified primary locations
- Flexibility to travel locally and within licensed states as needed
- Commitment to providing patient-centered care in diverse home environments
How to Stand Out (Preferred Qualifications):
- Multi-state licenses
- Experience in conducting in-home health evaluations
- Familiarity with telehealth practices and virtual patient engagement
- Strong communication and interpersonal skills
- Background in community health initiatives
