What to Expect (Job Responsibilities):
- Assess, diagnose, and determine treatment plans for patients using virtual health modalities
- Manage patient conditions with evidence-based care and appropriate interventions
- Provide health promotion and disease management counseling to patients and families
- Maintain accurate documentation in the electronic medical record and ensure compliance with coding practices
- Collaborate with interdisciplinary teams to facilitate coordinated patient care
What is Required (Qualifications):
- Active RN and applicable APRN/NP/APRN-FPA/APNP license(s) in state(s) of practice, or active PA license(s)
- National board certification as Nurse Practitioner or PA from relevant certifying bodies
- Active DEA registration and, if applicable, Illinois Controlled Substance License
- Master’s or Doctorate’s degree from accredited Nursing or PA Program
- Minimum of 3 years of relevant experience
