What to Expect (Job Responsibilities):
- Conduct virtual visits for Alignment members, performing comprehensive assessments
- Provide patient education on chronic conditions and self-care practices
- Facilitate patient empowerment by promoting informed choices regarding their care
- Collaborate in weekly interdisciplinary team meetings to develop appropriate care plans
- Ensure compliance with HIPAA regulations and maintain security of protected health information (PHI)
What is Required (Qualifications):
- Minimum of one (1) year of prior clinical experience
- Master’s degree from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
- Active Nurse Practitioner with Active RN Licensure or Physician Assistant Board Licensure
- Valid BLS (Basic Life Support) certification upon start
- Knowledge of current clinical standards of care and effective communication skills
How to Stand Out (Preferred Qualifications):
- Three years of prior clinical or home care experience
- Previous experience with Electronic Medical Records (EMR)
- Experience in the care of older adult (geriatric) patients
- Valid ACLS (Advanced Cardiovascular Life Support) certification
