      What to Expect (Job Responsibilities):
      - Answer all inbound calls to the virtual care center and utilize clinical judgment to address patient concerns
      - Collaborate with primary care physicians, Nurse Practitioners, and Case Managers to develop care plans for patients
      - Conduct outbound calls and virtual visits for patient follow-up
      - Review patient vitals daily for those enrolled in the remote patient monitoring program
      - Educate members on disease processes, medication compliance, and end-of-life topics

      What is Required (Qualifications):
      - Minimum of 1 year of experience as a Registered Nurse (RN)
      - Successful completion of an accredited Nursing Program; BSN preferred
      - Current and unrestricted RN license for California, Nevada, and North Carolina
      - CPR certification
      - Knowledge of clinical standards of care and effective communication skills with seniors

      How to Stand Out (Preferred Qualifications):
      - Experience in gerontology, adult care, palliative/hospice, or complex care management
      - Experience in Home Health, including wound care
      - Knowledge of Medicare Managed Care Plans
      - Bilingual skills, particularly in Spanish
      - EMR experience
