      What to Expect (Job Responsibilities):
      - Conduct a minimum of 25 in-home visits per week, including assessments, exams, and medication reviews
      - Manage chronic conditions and develop individualized treatment plans for patients
      - Serve as the primary care provider and conduct follow-ups to ensure continuity of care
      - Maintain accurate electronic medical record (EMR) documentation and comply with healthcare regulations
      - Collaborate with interdisciplinary teams to ensure coordinated patient care

      What is Required (Qualifications):
      - Active, unrestricted licensure as a Provider in the state of practice, clear of any clinical and/or board practice restrictions
      - Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant
      - Minimum of 1 year of experience in primary care or a related field such as family practice or outpatient care
      - DEA license and BLS certification
      - Strong clinical and diagnostic skills with proficiency in EMR systems

      How to Stand Out (Preferred Qualifications):
      - Experience with risk adjustment models for the Center for Medicare and Medicaid Services (CMS) and the Affordable Care Act (ACA)
      - Demonstrated empathy and respect for members during in-home assessments
      - Strong computer skills and knowledge of electronic medical records
