      What to Expect (Job Responsibilities):
      - Conduct in-home assessments, medication reviews, and health screenings for patients
      - Provide patient education by aligning care desires with optimal care solutions
      - Identify diagnoses for care management and active medical management
      - Collaborate with the Regional Medical Officer and team members to develop appropriate care plans
      - Coordinate care with multiple stakeholders, including primary care providers and specialists

      What is Required (Qualifications):
      - Master's degree from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
      - Active Nurse Practitioner or Physician Assistant license
      - Active Nurse Practitioner Board Certification or Physician Assistant Certification
      - Valid NPI Number, DEA, and BLS certification
      - Minimum of one (1) year of prior clinical or home care experience preferred

      How to Stand Out (Preferred Qualifications):
      - Experience in the care of older adult (geriatric) patients preferred
      - Previous EMR experience preferred
      - Strong problem-solving skills and the ability to work in a fast-paced environment
      - Excellent communication skills, both professionally and effectively
