What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews
- Engage with patients to answer health questions and provide personalized care
- Utilize technology for clinical workflows and documentation during evaluations
- Travel locally and within licensed states as needed for patient visits
- Conduct virtual visits when required
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Ability to cover primary locations, including Bronx and surrounding areas
- Previous experience in providing in-home or community-based healthcare
How to Stand Out (Preferred Qualifications):
- Multi-state licenses preferred
- Experience working with diverse patient populations
- Familiarity with diagnostic and preventive services
