      What to Expect (Job Responsibilities):
      - Conduct in-home assessments on patients, including medication reviews and health screenings
      - Provide patient education by aligning desired care with the best care options available
      - Identify diagnoses for care management and active medical management
      - Collaborate with the Regional Medical Officer and team members to develop tailored care plans
      - Coordinate care with various stakeholders, including primary care physicians and specialists

      What is Required (Qualifications):
      - Master's degree from an accredited Nurse Practitioner or Physician Assistant program
      - Active Nurse Practitioner or Physician Assistant license
      - Active RN License and Furnishing number (for NP only), along with Board Certification
      - Effective communication, problem-solving, organizational, and time management skills
      - Valid Driver's License and current automobile insurance

      How to Stand Out (Preferred Qualifications):
      - One (1) year of prior clinical or home care experience
      - Previous experience with electronic medical records (EMR)
      - Experience in geriatric patient care
