      What to Expect (Job Responsibilities):
      - Conduct in-home assessments and some outpatient clinical work for health plan members
      - Perform comprehensive history taking, physical exams, medication reviews, and cognitive/depression/safety screenings
      - Engage in end-of-life care discussions with patients and families
      - Collect laboratory specimens in the home setting when appropriate
      - Communicate assessment findings to inform primary care providers of potential gaps in care

      What is Required (Qualifications):
      - Master's degree from an accredited Nurse Practitioner or Physician Assistant program
      - Active California state Nurse Practitioner or Physician Assistant license
      - Active RN License and Furnishing number (for Nurse Practitioners)
      - Active Nurse Practitioner Board Certification/Physician Assistant Certification
      - Ability to communicate positively and effectively with others

      How to Stand Out (Preferred Qualifications):
      - One (1) year of prior clinical or home care experience
      - Previous experience with electronic medical records (EMR)
      - Experience in the care of older adult (geriatric) patients
