      What to Expect (Job Responsibilities):
      - Provide skilled physical therapy to patients/clients with various diagnoses and disabilities under the supervision of a Physical Therapist
      - Customize treatment plans to meet individual patient needs and goals
      - Foster relationships with a diverse patient population in their home settings
      - Collaborate with healthcare team members to enhance patient independence and functional potential
      - Document patient progress and treatment outcomes accurately and timely

      What is Required (Qualifications):
      - Associate's Degree in Physical Therapy
      - Oregon Physical Therapy Assistant License upon hire
      - National Provider BLS certification from the American Heart Association upon hire
      - Strong skills in patient/client treatment and professional consultations
      - Ability to accommodate individual patients' special physical or age-related needs

      How to Stand Out (Preferred Qualifications):
      - Experience working in home health or with diverse patient populations
      - Familiarity with electronic health records and documentation processes
      - Additional certifications in specialized areas of physical therapy
