What to Expect (Job Responsibilities):
- Deliver personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities
- Conduct comprehensive assessments and develop tailored care plans for older adults
- Manage chronic conditions and provide empathetic care for patients and their families
- Collaborate with cross-functional partners to deliver coordinated, whole-person care
- Build trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals
What is Required (Qualifications):
- Active, unencumbered NP or PA license in the state of practice
- Minimum of 1-2 years of clinical experience, preferably in geriatrics or primary care
- Compassionate, patient-centered approach with strong clinical judgment
- Valid driver's license and reliable transportation
How to Stand Out (Preferred Qualifications):
- Experience in dementia-informed care or working with older adults
- Strong communication and interpersonal skills
- Previous experience in a collaborative healthcare environment
