What to Expect (Job Responsibilities):
- Direct the coordination of care under the supervision of the Executive Director
- Schedule client appointments and visits according to care plans and staff availability
- Maintain communication with care providers, clients, and staff regarding day-to-day changes
- Supervise the scheduling of appropriately skilled personnel to fill cases
- Perform general office duties including word processing, filing, and handling telephone referrals
What is Required (Qualifications):
- At least two years of experience as a Coordinator
- Proficiency in utilizing a PC, calculator, and multi-line telephone
- Strong communication, negotiation, and public relations skills
- Demonstrated organization, assertiveness, and flexibility in job responsibilities
- Ability to adhere to company attendance and reliability guidelines
How to Stand Out (Preferred Qualifications):
- Experience in home health care or hospice services
- Knowledge of case management processes and procedures
- Familiarity with medical terminology
- Skills in conflict resolution and problem-solving
- Previous experience in a supervisory role
