      What to Expect (Job Responsibilities):
      - Conduct inbound and outbound calls to members and providers to facilitate access to healthcare services
      - Assess members' health status using motivational interviewing and healthcare knowledge
      - Engage members to promote behavior changes and optimal health outcomes
      - Collaborate with members and providers to identify and integrate available resources and programs
      - Develop proactive strategies to address health-related issues and enhance member wellness

      What is Required (Qualifications):
      - 5+ years of practice experience in Nursing, Renal, Diabetes, Hypertension, Primary Care/Geriatric, or Population Health
      - 3+ years of managed care or Medicare experience
      - 3+ years of experience with Microsoft Office (Outlook, Word, Teams, Excel, OneNote)
      - Must be technologically proficient and able to learn new systems quickly
      - Associate's Degree or equivalent experience

      How to Stand Out (Preferred Qualifications):
      - Healthcare licensure and/or certifications (RN, LPN, RD, MSW, LCSW, CDE, etc.)
      - Bilingual (Spanish)
      - Previous experience in telephonic coaching, education, or case management
      - Familiarity with behavioral change and motivational interviewing techniques
