What to Expect (Job Responsibilities):
- Conduct telephonic outreach to Medicare members to improve medication adherence and health outcomes
- Utilize motivational interviewing and assessment tools to evaluate members' health status
- Collaborate with members to identify barriers and integrate them into available programs
- Provide coaching and support to empower members in making informed healthcare decisions
- Develop proactive action plans to enhance members' overall wellness and health outcomes
What is Required (Qualifications):
- 1-3 years of experience with Microsoft Office
- 3-5 years of practice experience in Renal, Diabetes, Hypertension, Primary Care/Geriatric, or Oncology
- Experience in Managed Care and Medicare
- Associate's Degree or equivalent experience
- Proficient in utilizing healthcare knowledge to address member needs
How to Stand Out (Preferred Qualifications):
- Healthcare licensure and/or certifications (RN, LPN, MSW, LCSW, CDE, etc.)
- 5 years of practice experience in relevant fields
- Previous experience in telephonic coaching, education, or case management
- Excellent analytical, problem-solving, and communication skills
- Bilingual proficiency
